In just a few minutes, you can discover your risk of having a heart attack or stroke in the next 10 years, and uncover goals to lower that risk by taking the My Diabetes Health Assessment.
Step 1 Take a minute to answer a few questions to estimate your current risk of heart attack or stroke.
Step 2 View your risk results report to compare the current levels of your health numbers, like cholesterol and blood glucose, to your recommended goal levels.
Step 3 Discover what you can do to reduce your risk of heart attack and stroke by setting goals like stopping smoking, increasing physical activity or losing weight.
Step 4 Print your customized action plans, selected for you based on current risk levels, and take them to your next healthcare appointment to discuss with your provider.
Step 5 Enroll in The Heart of Diabetes™ program to help you stay informed, inspired and involved in managing your type 2 diabetes and reaching your goal levels.
